Ce pays où le généraliste impose sa loi à l'hôpital
Le 1er avril dernier est entrée en vigueur une importante réforme du système de santé anglais. Dans un contexte de rigueur sans précédent, la moitié du budget alloué à la santé est confié en gestion aux médecins généralistes, chargés de réguler le recours aux soins hospitaliers.
Le 27 juillet 2012, une scène de la cérémonie d’ouverture des Jeux Olympiques de Londres a interloqué des millions de spectateurs étrangers : une armée de nurses (les infirmières anglaises) et d’enfants ont batifolé au milieu du grand stade, avant que trois lettres lumineuses ne l’illuminent : NHS, pour National health service. Le metteur en scène Danny Boyle a marqué les esprits, car les Britanniques sont viscéralement attachés à leur système de santé, aussi constitutif de leur identité nationale que la royauté. Créé en 1948, le NHS est universel et gratuit. Entièrement financé par l’impôt, il emploie 5,6% de la population active anglaise (depuis 1999, les NHS gallois, écossais et nord-irlandais sont gérés indépendamment). Si le NHS a été si fiévreusement célébré, c’est aussi parce qu’une partie des Anglais le croient menacé.
Généralistes, "porte d'entrée du système"
Le 1er avril 2013 est entrée en vigueur la plus importante réforme de structure que cette vieille institution n’ait jamais connue. Le gouvernement de coalition entre conservateurs et libéraux-démocrates, dirigé par le Premier ministre David Cameron, veut "changer sa culture" : alléger sa "bureaucratie", en coupant 30% des dépenses administratives, et le mettre en concurrence, afin de la rendre plus efficace. Le tout dans un contexte d’austérité inconnu sous nos latitudes : pour faire face à la progression naturelle des dépenses de santé d'environ 4 % par an, il doit donc réaliser environ 20 milliards de livres (25 milliards d'euros) d'économies en 5 ans.
Les médecins généralistes sont les principaux artisans de cette réforme. Ils étaient déjà la "porte d’entrée" du système : les anglais doivent choisir leur médecin généraliste traitant (general practitioner dit GP) dans une zone géographique restreinte. L’accès aux spécialistes, qui sont tous à l’hôpital, passe obligatoirement par les médecins généralistes. Ceux-ci sont rémunérés au forfait, selon une équation complexe prenant en compte le nombre de leurs patients, l’état de santé de la population locale et le coût de la vie locale. Les anglais sont ainsi les pionniers de l’organisation des soins primaires.

The country where GPs rule over hospitals
On 1st April, an important reform of the English healthcare system came into effect. In the context of unprecedented austerity, half of the budget allocated to health has been put in the hands of general practitioners, who are charged with regulating access to hospital care.
On 27th July 2012, millions of foreign spectators were taken aback by a scene in the opening ceremony of the London Olympic Games: an army of nurses and children frolicked in the middle of the main stadium before three neon letters lit up: NHS (National Health Service). The director Danny Boyle made an impact: the British people are deeply attached to their health system, which is just as much a part of their national identity as royalty. Created in 1948, the NHS is universal and free. Entirely financed through taxation, it employs 5.6% of the English working population (since 1999, the Welsh, Scottish and Northern Irish NHS have been independently managed). If the NHS has been fiercely celebrated, it is also because some English people believe it to be under threat.
GPs, ‘gateway to the system’
On 1st April 2013, the most important structural reform that this long-standing institution has ever known came into effect. The Conservative–Liberal Democrat coalition government, led by Prime Minister David Cameron, wants to ‘change its culture’: to reduce its ‘bureaucracy’ by cutting 30% of administrative costs, and to make it more competitive, in order to make it more efficient. All this in the context of an austerity unknown in our society: in order to cope with the normal rise in health expenditure of about 4% a year, 20 billion pounds (25 billion euros) will need to be saved in 5 years.
GPs are the key players in these reforms. They were already the ‘gateway’ to the system: English people have to choose their general practitioner (GP) within a restricted geographical area. Access to specialists, who are all hospital-based, has to pass through general practitioners. The latter are paid a flat rate, according to a complex calculation involving the number of patients they have, the local population’s state of health and the local cost of living. This makes the English pioneers in the organisation of primary care.

Source text - Spanish España no se queda al margen de la necesidad de mayor cooperación en la gestión del agua, que Naciones Unidas ha identificado como herramienta clave para resolver los problemas del agua en el mundo. WWF pide a las administraciones una mayor apertura a la participación pública de los españoles, apoyada en una mayor transparencia, como vía para resolver los problemas de escasez de agua del país.
2013 ha sido declarado por la ONU como año Internacional de la Cooperación en la Esfera del Agua. Una cooperación que debe darse no sólo entre gobiernos de diferentes países, sino entre todos los actores implicados en la gestión de este valioso recurso: gobierno, organizaciones internacionales, sector privado, sociedad civil y académica.
La cooperación entre actores surge como parte imprescindible de la solución frente a las 4 amenazas globales que ha identificado la ONU en materia de agua, y a las que España no es ajena:
- Sobreexplotación de los recursos: cada año se usan en el mundo 3.800 km3 de agua dulce. España consume más de 2,2 km3 al año.
- Derivación: el agua de los ríos se embalsa y se deriva para el uso humano. Mientras en 1950 había 500 grandes presas en el mundo, en 2013 hemos alcanzado las 45.000.
- Escasez: consumimos más agua de la que hay disponible. En 2030 se estima que el 47% de la población del mundo vivirá en zonas de alto estrés hídrico por sobreconsumo. España ya sufre la escasez de agua en más de la mitad de su superficie.
- Abandono de los tratados internacionales: se acuerdan unas reglas del juego, pero luego se incumplen, lo que genera conflictos internacionales. En la península Ibérica no sólo se incumple el tratado de Albufera con Portugal, sino que se incumple la propia legislación de aguas.
Es indudable que la gestión del agua en España debe mejorar, pero esto no se consigue construyendo más infraestructuras de regulación, sino mejorando el uso del agua que hay disponible. Según Eva Hernández de WWF, “es imprescindible mejorar la gobernanza del agua en España, asumir que los ciudadanos somos mayores de edad y que podemos contribuir a la toma de decisiones, si la Administración aporta el conocimiento científico necesario y es transparente en su información”.
Translation - English Spain does not remain on the sidelines with regard to the need for greater cooperation in the management of water, which the United Nations has identified as a key tool for resolving the world’s water problems. The WWF is asking that the government adopt a greater openness to the participation of the Spanish public, based on greater transparency, as a way of resolving the country’s water shortage problems.
The UN has declared 2013 the International Year of Water Cooperation. A cooperation that must take place not only between governments of different countries but also between all the actors involved in the management of this valuable resource: governments, international organisations, the private sector, civil society and academic communities.
Cooperation between actors is a vital part of the solution to the four global water threats that the UN has identified, and to which Spain is no stranger:
- Over-exploitation of resources: each year the world uses 3,800 km3 of fresh water. Spain consumes more than 2.2km3 per year.
- Diversion: river water is collected in reservoirs and diverted for human use. Whilst in 1950 there were 500 large reservoirs in the world, in 2013 we have reached 45,000.
- Shortage: we consume more water than is available. It is estimated that in 2030 47% of the world’s population will live in areas of serious water stress owing to over-consumption. More than half the surface area of Spain already suffers water shortage.
- Abandonment of international treaties: the rules of the game are agreed but are then breached, which leads to international conflict. In the Iberian Peninsula not only has Albufera’s treaty with Portugal been breached but also its own water legislation.
There is no doubt that water management in Spain needs to improve, though this is not achieved by building more regulatory infrastructure but by improving use of the water that is available. According to Eva Hernández of the WWF, ‘it is essential to improve water management in Spain, to accept that our citizens are adults and can contribute to decision making if the Government provides the necessary scientific knowledge and is transparent in its provision of information’.

Le lieutenant Rouyer, du 48ème Régiment d’Infanterie breton qui combattit notamment dans le Bois de Ham-sur-Sambre, particpait aux cérémonies du Souvenir et, en 1952, il donna une conférence sur ses souvenirs de guerre. Ecoutons-le :
«Nous avions assisté, la nuit, au lugubre spectacle des incendies et de l’exode en panique de femmes, de vieillards et d’enfants. Or, voici soudain, un de nos hommes découvre un petit bébé, inconscient du danger sous les rafales qui atteignaient tant de nos camarades. Il l’amène au colonel qui l’embrassa avec tant de délicatesse que pour nous ses soldats, presque tous pères de famille, ce geste fit plus que la sonnerie à la charge ! La petite fille fût recueillie et soignée par la famille où j’avais logé la veille.
Âgée de quelques mois, sa maman l’avait emportée, serrée dans son tablier. Dans sa fuite éperdue, elle l’avait perdue… Elle ne sut jamais, de toute la guerre, ce qu’elle était devenue, sans doute la croyant morte, dans de telles conditions ! Un peu après la guerre, elle apprit qu’une enfant trouvée avait été recueillie dans une famille 'Drapier' à Fosses. On sut ainsi son nom : Noëlla Genot, née à Ham-sur-Sambre le 6 décembre 1913. « Mimi des Bois » fut officiellement adoptée par Emile Drapier en 1939, devenant Noëlla Genot-Drapier.
Elle entra chez les sœurs de Saint-Vincent-de-Paul à Ans, ces religieuses soignantes aux grandes cornettes. Après une vie au service des malades en diverses localités, elle mourut à Ans vers 1995.

Lieutenant Rouyer, of the 48th Breton Infantry Regiment, who fought notably in Ham-sur-Sambre woods, took part in the Memorial Ceremonies and, in 1952, gave a talk on his war memories. Let's listen to him:
"We had witnessed, during the night, the bleak spectacle of fires and the panicked flight of women, the elderly and children. But, all of a sudden, one of our men discovered a little baby, unaware of the danger beneath the gunfire that reached so many of our fellow soldiers. He took it to the Colonel, who kissed it so gently that for we soldiers, almost all fathers, the gesture affected us more than the call to battle. The little girl was collected and looked after by the family I had stayed with the night before.
A few months old, her mother had carried her away held tight in her apron. In her frantic flight, she had lost her... She never knew, throughout the whole of the war, what had become of her, doubtless believing her dead under such circumstances! A little after the war, she learnt that a child who had been found had been taken in by a family called 'Drapier' in Fosses. This is how her name was discovered: Noëlla Genot, born in Ham-sur Sambre on 6th December 1913. "Mimi of the Woods" was officially adopted by Emile Drapier in 1939, becoming Noëlla Genot-Drapier.
She joined the Sisters of Saint-Vincent-de-Paul at Ans, those nuns with the large cornet wimples who cared for the sick. After a life in the service of the sick in various locations, she died in Ans around 1995.

Dès le samedi 22 août 1914, en soirée, les troupes françaises repassent vite, hélas, en désordre, talonnées par l’ennemi. De nombreux blessés sont ramenés du combat et soignés chez les Sœurs de Sainte-Marie, à Saint-Martin, au château Destrée («Le Chapitre »), puis dans les écoles, au presbytère et chez le vicaire.
Durant l'afflux de blessés les trois premiers jours, il n'y avait aucun médecin pour soigner 300 blessés ! Les premiers sont arrivés déjà le vendredi 21 août. Mais, sous la direction du doyen Crépin, des équipes de la Croix Rouge dont 14 brancardiers et veilleurs de nuit, 24 dames et demoiselles infirmières, nettoyeuses, blanchisseuses, cuisinières, et 25 religieuses de Sainte-Marie et de Saint-Martin (françaises) avaient prévu et organisé 5 implantations hospitalières.
Ce n'est que le 25 ou le 26 que les Allemands ont envoyé 3 puis encore 4 médecins militaires belges (en principe prisonniers), un pharmacien, un officier d'administration et un médecin allemand.
La secrétaire de la Croix Rouge a ouvert un registre où elle a consigné les noms, prénoms, n° de régiment et Compagnie, et une adresse de parents, pour 216 soldats. Mais elle n'a pas pu tout inscrire tellement ils affluaient ! Il y eut 29 décès, dont deux officiers) inhumés au Vieux Cimetière.
Et aussi une trentaine d'allemands, plus 70 blessés à l'école de Vitrival dont s'est occupé aussi le doyen Crépin.

From Saturday 22nd August, in the evening, the French troops quickly returned, alas, in disarray, with the enemy at their heels. Many injured were brought back from combat and cared for by the Sisters of Sainte-Marie in Saint-Martin, in Destrée Château ("The Chapter"), then in schools, at the presbytery and the clergy house.
During the influx of the injured on the first three days, there was no doctor to care for the 300 injured! The first had already arrived on Friday 21st August. But, under the direction of Dean Crépin, the Red Cross teams, of whom 14 were stretcher bearers and night watchmen, 24 female nurses, cleaners, laundrywomen and cooks, and 25 nuns from Sainte-Marie and Saint-Martin (French), had made provision for and organised 5 hospital installations.
It wasn't until the 25th or 26th that the Germans sent 3, then another 4, Belgian military doctors (essentially prisoners), a pharmacist, an administrative officer and a German doctor.
The Red Cross secretary created a register where she recorded the first and surnames, regiment and company number, and parental address for 216 soldiers. But so many poured in that she wasn't able to record them all! There were 29 dead, of whom two were officers, buried in the Old Cemetery.
And also about thirty Germans, plus 70 injured at Vitrival School, which was also looked after by Dean Crépin.

Source text - French Sart Eustache Chateau
Le soir du 21 août, 2 bataillons du 25e R. I. de Cherbourg bivouaquent à Sart-Eustache. Le lendemain, ils iront au combat à Presles puis à Le Roux. Ce jour-là, les habitants quittent rapidement le village.
A 10h du matin, les premiers blessés français arrivent au Sart; ils sont conduits au château où le curé Servais les acceuille avec des volontaires restés au village.
L’ambulance de Sart-Eustache est installée dans le château. Elle vient de Fougères en Bretagne et se compose de six médecins, un pharmacien, des infirmiers, sept voitures de chirurgie, pharmacie, etc. On a garni le rez-de-chaussée du château de matelas sur lesquels s’allongent des officiers, des soldats en sang, certains méconnaissables…
Les troupes françaises et l’ambulance quittent le village le 22 août.
Les premiers allemands parviennent au château où le curé Servais se trouve seul, au milieu des blessés : «Je vis arriver les premiers allemands, conduits par un officier. (...) Il se mit alors à visiter tout le château. Partout, je dus le précéder. Il me fut permis de passer la nuit au milieu des blessés français, tandis que les allemands s’enivraient.»
Dès le matin du 23 août, des combats vont opposer les soldats français aux allemands. Le lieutenant Souvré tombe parmi les premiers et six autres français périront en s’enfuyant par les Ruelles. Deux autres enfin seront retrouvés morts dans le village.
Après la guerre, les corps des soldats bretons furent inhumés dans le cimetière communal et l’on plaça une plaque commémorative.

On the evening of 21st August, 2 battalions of the 25th Cherbourg Infantry Regiment bivouacked at Sart-Eustache. The following day they would go to battle at Presles and then at Le Roux. That day, the inhabitants quickly left the village.
At 10 in the morning, the first injured French arrived at Sart; they were taken to the château where Priest Servais welcomed them with the volunteers who had stayed in the village.
Sart-Eustache first-aid post was installed in the château. It came from Fougères in Brittany and was composed of six doctors, a pharmacist, nurses, seven surgical carriages, a pharmacy, etc. The château basement was furnished with mattresses on which officers and bloodied soldiers - some unrecognisable - lay outstretched...
The French troops and the first-aid post left the village on 22nd August.
The first Germans reached the château where Priest Servais was alone, in the midst of the injured: "I saw the first Germans arrive, led by an officer. (...) He then set about looking round the whole château. I had to lead him everywhere. I was allowed to spend the night in the middle of the injured French whilst the Germans got drunk."
From the morning of 23rd August, the battles would pit the French soldiers against the German. Lieutenant Souvré fell amongst the first and six other French perished fleeing via Les Ruelles. Two others would finally be found dead in the village.
After the war, the bodies of the Breton soldiers were buried in the municipal cemetery and a commemorative plaque was placed there.
