מצב אחרי קרניוטומיה להסרה של מדולובלסטומה מהגומה האחרוית לפני כשבועיים.
גוש על הזיז החולייתי בגובה T8, לא ניתן לסיווג.
החלמה כללית טובה אך איטית. עדיין סובל מאטקסיה. חסך ברור בתנועת הראש, כנראה משני לכאב. ללא פסאודומינינגוצל במיטת הניתוח, הפצע מחלים בצורה יחסית טובה.
בדיקה גופנית -
תנועות עיניים בגדר הנורמה.

Translation - English Current Illness
Status post craniotomy for removal of medulloblastoma from the posterior fossa approx. two weeks ago.
Lump on the spinous process at T8 area, we cannot yet classify.
Good general recovery, but slow. Still suffering from ataxia. Distinct head movement deficiency, probably secondary to the pain.
No pseudomeningocele on the operated area, the wound is healing rather well.
Physical –
Normal eye movement.
Summary -
Several points that require reference:
Oncology – biopsy of the lesion in T8? I explained the technical meanings to the family.
If we continue in this path, it should be done with the oncology team.
Eyes – I request an eye examination by the neuro-ophthalmology team during the following days.
Gait deficiency – should improve gradually and it is connected to the recent surgery.
Physiotherapy is required and probably rehabilitation process, this matter is connected to the oncological points.

Source text - Hebrew ביקרנו את המאזנים המצורפים של (להלן - החברה) לימים 31 בדצמבר 2020 ואת דוח התוצאות העסקיות לשנה שהסתיימה באותם תאריכים. דוחות כספיים אלה הינם באחריות הדירקטוריון
וההנהלה של החברה. אחריותנו היא לחוות דיעה על דוחות כספיים אלה בהתבסס על ביקורתנו.
הדוחות הכספיים ליום 31 בדצמבר 2019 ולשנה שהסתיימה באותו תאריך בוקרו על ידי רואה חשבון מבקר אחר אשר הדוח שלו עליהם מיום 30 ביולי 2020 כלל חוות דעת בלתי מסויגת.
ערכנו את ביקורתנו בהתאם לתקני ביקורת מקובלים בישראל, לרבות תקנים שנקבעו בתקנות רואי חשבון (דרך פעולתו של רואה חשבון), התשל"ג-1973. על-פי תקנים אלה נדרש מאיתנו לתכנן את הביקורת ולבצעה במטרה להשיג מידה סבירה של ביטחון שאין בדוחות הכספיים הצגה מוטעית מהותית. ביקורת כוללת בדיקה מדגמית של ראיות התומכות בסכומים ובמידע שבדוחות הכספיים. ביקורת כוללת גם בחינה של כללי החשבונאות שיושמו ושל האומדנים המשמעותיים שנעשו על ידי הדירקטוריון וההנהלה של החברה וכן הערכת נאותות ההצגה בדוחות הכספיים בכללותה. אנו סבורים שביקורתנו מספקת בסיס נאות לחוות דעתנו.

Translation - English We have audited the attached general balance of (hereinafter - the company) for the period ending on December 31, 2020 and the transactions cycle for the year ending on the above date. These financial reports are under the exclusive responsibility of the company’s directorate and management. Our responsibility includes an audit report based on our opinion as auditors.
The financial reports as of December 31, 2019 and for the year ending on this date have been audited by another auditor accountant and his/her report regarding the above on July 30, 2020 includes an unqualified opinion.
We have conducted our audit according to the standard and conventional reporting standards in Israel, including standards determined by the Auditors’ Standards (Auditor’s Course of Action), 5733 - 1973. According to these standards, we are required to outline and conduct the audit in order to verify and affirm that the financial reports do not contain substantial false information. The audit includes a random check of evidence that supports the specified amounts and information in the financial reports. An audit also includes the applied calculation methods, the declared gross estimations by the company’s directorate and management, and assessment of the general presentation of the information in the documentation. We believe our audit constitutes an adequate foundation for our opinion.
As mentioned in annotation 2 in the financial reports, the financial reports are specified in nominal values. Information regarding the impact of the changes to the general purchasing power in the Israeli currency on the financial reports, as specified in the declaration of opinion by the Institute of Certified Public Accountants in Israel, is not included in these financial reports.
In our opinion, apart from excluding the specified information in the last paragraph, the financial reports below constitute an accurate reflection, in accordance with the conventional reporting standards and with respect to all the substantial factors, of the company’s financial situation as of December 31, 2020 and the results of the company’s activities in the year that ended on the above date, in accordance with the reporting standards.
