Source text - English CARDIOGENIC SHOCK
The incidence is decreasing since the advent of thrombolytic agents.
Causes
Anything that prevents the forward flow of blood from the heart, including:
• ● Massive myocardial necrosis following infarction where 40% or more of the ventricular mass has been lost;
• ● Ventricular septal defect;
• ● Papillary muscle rupture;
• ● Pericardial tamponade;
• ● Ventricular aneurysm;
• ● Pulmonary embolism;
• ● Tachy or bradyarrhythmias;
• ● End stage congestive/ischaemic cardiomyopathy; and
• ● Traumatic myocardial contusion.
Loss of myocardium causes decreased contractility.
Definition: Systolic Blood Pressure < 90mmHg or MAP < 65 for > 30 minutes or inotropes are required to achieve this. Accompanied by - Pulmonary congestion, Impaired organ perfusion, cold clammy skin, decreased urine output, altered mental state, and an elevated serum lactate.
Compensatory Mechanisms
• ● Tachycardia; and
• ● Increased systemic vascular resistance – vasoconstriction, pallor, decreased renal blood flow.
MANAGEMENT
1. Oxygenate;
2. Analgesia;
3. Control of arrhythmias;
4. General principles as outlined in ‘Introduction to Shock’;
5. Positive inotropes: Consider starting with Noradrenaline and add Milrinone or
Dobutamine; and
6. ? use of Left heart assist devices (intra-aortic balloon pump) and PCI
