Job applicant questionnaire with individual items to fill out or mark. As far as children/dependents are concerned, I seem to be running into "danove zvyhodneni", not "mzdove". Is it something like a "child/dependent credit" in the US or what would be the proper term?

not claiming tax allowance
the context above is not very clear but I think the child tax allowance is automatically calculated into the monthly pay of a person who is claiming it--that's why it may be called "mzdové" in this case.

tax relief/deductible
I have never heard about any wage inscentives like that. Sure, it's possible that an employer would pay you more if you have kids (even though I don't understand why the heck they would do it) - but even if so, they would not need any declaration from your spouse. No, this is just the standard tax relief - just the guy does not know what he is talking about.

: perhaps I should ask for a wage incentives for my two dogs - the more dogs, the bigger tax relief :-)))))

not claiming Child Tax Credit
Here in the UK, it is called Child Tax Credit. Child Tax Credit is for people who are responsible for at least one child or qualifying young person. Child Tax Credit is paid direct to the person who is mainly responsible for caring for the child or children. If you are a lone parent you will receive the payment. Child Tax Credit can be paid to workers who continue to pay UK National Insurance Contributions when posted from the UK to work in another country in the European Economic Area.
This is similar to Working Tax Credit. Working Tax Credit is for people who are employed or self-employed (either on their own or in a partnership), who
usually work 16 hours or more a week
are paid for that work, and
expect to work for at least 4 weeks
For more details, please see below :-)

child benefit
tu je jeden tip ktory som nasiel v ramci vestnika EU:
tu je text z Reference for a preliminary ruling from the Finanzgericht Köln
Is Article 10 of Council Regulation (EEC) No 574/72 of 21 March 1972 for implementing Regulation (EEC) No 1408/71 [2] to be interpreted restrictively, so as not to prevent entitlement to child benefit of a mother bringing up children on her own in the State of residence (Federal Republic of Germany), who does not receive CHILD BENEFIT in the State of employment (Kingdom of the Netherlands) because of the age of the children?
