"I got the feeling that this sentence should be understood as the pay for preventive service is not included or accumulated when we consider the maximum pay of a covered year."
THIS SEEMS CORRECT!

: Your interpretation is correct but the answer is wrong. 'Taken in a negative way' means something completely different.

Discussion
Or maybe I should use a neutral expression. Count against may mean "affect" or "influence".
I got the feeling that this sentence should be understood as the pay for preventive service is not included or accumulated when we consider the maximum pay of a covered year.
