Tax ID number?

See http://www.yadeliezer.org/tax-deduction
Perhaps it is an abbreviation for מספר העמותה

What does the number look like? How many digits? etc.

I have no idea what any of it means. The Yad Eliezer example cited above has 9 digits and begins with 580. Israeli ID numbers have 9 digitis. You could try googling מלכ"ר מ.ה followed by the number in your document and see what you come up with. If you don't find anything, you could just write "Non-profit number followed by the five digits. I don't think it makes sense to guess beyond that if you really don't know what to call that number (ID, Tax ID, Amuta, Registration Number, Association Number, etc. Good luck!
Example sentence:

View our teudat amuta (registered Israeli non-profit status) here.

Note from asker:
Thanks, Mary. The number starts with 589, and has 9 digits all cramped together, no separators. I don't want to guess what the first digits indicate. Do you have any idea?

מס הכנסה / Income Tax

Given that this is an invoice (according to the subject field)...Could it be a case of Occam's razor: the simplest explanation is usually the right one?

In this case "income tax [number]".

Guessing, hence the confidence level.

Agav, I'm aware it's a non-profit organization, but on occasion they still may be liable for tax and might require a tax number for such occasions.
Note from asker:
could well be. thanks.
Peer comment(s):

neutral Sandra & Kenneth Grossman : This is not a likely pattern. מ probably means number.

I know, I just thought as mispar was written after it, there was an outside chance it could be something else. Unlikely, but possible (but reflected in the confidence level). Thanks Sangro!

Association number

מספר התאגדות
Makes sense in this context and widely used.

Reg. No.

I am guessing that this is the registration no. in מרשם העמותות.
The abbreviation "מ"ה" not infrequently stands for "mispar ha... whatever", according to the logic of the secretary who typed the form.

The reg. number is reqd on all forms (unlike the tax ID which, incidentally, may well be identical).
