Looking at the whole text provided - I believe you are right - as thrombomodulin is expressed in the cell membrane.

cancer foci As far as I understand this, "cancer nuclei" is referring to the actual nuclei of cancerous cells, i.e. within the cells. The meaning of "cancerhärdar" (with an "a"), however, appears to be more accurately reflected by "cancer foci" or simply "tumours", as it refers to a microscopically visible mass of (many) cancerous cells.

This must have come up for me once because I found it somewhere and wrote it down in my copy of Cressy for future reference (please note that I'm not a doctor or anything so I do stand to be corrected if someone who's worked in the medical profession knows better).
