INTENDED USE
Enzyme linked immunosorbent assay (ELISA) for the qualitative and semi-quantitative detection of antibodies to oxidized low-density lipoprotein (oxLDL) in human serum.
SUMMARY AND EXPLANATION
Anti-oxidized LDL antibodies are associated with certain clinical conditions such as atherosclerosis, carotid stenosis and other coronary artery dysfunction. Anti-oxidized LDL antibodies may also be significant in diseases such as diabetes hypertension and endometriosis.1-5
Hypercholesterolemia is closely associated with increased risk of atherosclerosis.4,6 Cholesterol accumulated in the atherosclerotic plaque is derived primarily from low-density lipoprotein (LDL). Oxidation of LDL is accepted as a critical event in the atherogenic process.7 Free radicals like superoxide and nitric oxide (-O2, NO) generated in biological reactions in the body contribute to the oxidation of LDL.8,9 The NO radical has been shown to oxidize apolipoprotein B, a constituent of LDL. Similarly, lipo-oxygenases and oxidants like per-oxynitrite can oxidize the lipid moieties in LDL. It has been demonstrated that oxidized LDL and not native LDL is taken up by scavenger receptors on monocytes, smooth muscle cells and macrophages in the blood vessels. The oxidation of LDL increases the affinity of oxidized LDL to acetyl receptors of macrophages.10 As this pathway of oxidized LDL uptake is unregulated, it results in the formation of lipid-laden macrophages (foam cells). The presence of foam cells is the earliest step in the formation of atherosclerotic plaque in blood vessels. Later, other inflammatory molecules released by leukocytes promote the progression of atherosclerotic plaque.11,12
Antibodies against oxLDL and its complexes have been demonstrated in systemic lupus erythematosus, diabetes, arterial thrombosis, hypertension, etc.
PRINCIPLES OF PROCEDURE
The test is performed as a solid phase immunoassay. Microwells are coated with oxLDL antigen followed by a blocking step to reduce non-specific protein binding during the assay run. Controls, calibrators and patient sera are incubated in the antigen coated wells to allow specific antibodies in samples to bind to the antigen. Unbound antibodies and other serum proteins are removed by washing the microwells. Bound antibodies are detected by adding an enzyme labeled anti-human IgG conjugate to the microwells. Unbound conjugate is removed by washing. Specific enzyme substrate (TMB) is then added to the wells and the presence of antibodies is detected by a color change produced by the conversion of TMB substrate to a colored reaction product. The reaction is stopped and the intensity of the color change, which is proportional to the concentration of antibody, is read by a spectrophotometer at 450 nm. Results are expressed in ELISA units per milliliter (EU/ml) and reported as positive or negative.
REAGENTS
Storage and Preparation
Store all reagents at 2-8°C. Do not freeze.
Do not use if reagent is not clear or if a precipitate is present. All reagents must be brought to room temperature (20-25°C) prior to use.
Reconstitute the wash buffer to 1 liter with distilled or deionized water. When stored at 2-8°C, the reconstituted wash buffer is stable until the kit expiration date. Coated microwell strips are for one time use only.
Precautions
All human derived components used have been tested for HBsAg, HCV, HIV-1 and 2 and HTLV-I and found negative by FDA required tests. However, human blood derivatives and patient specimens should be considered potentially infectious. Follow good laboratory practices in storing, dispensing and disposing of these materials.13
Instructions should be followed exactly as they appear in this kit insert to ensure valid results. Do not interchange kit components with those from other sources. Follow good laboratory practices to minimize microbial and cross contamination of reagents when handling. Do not use kit components beyond expiration date on the labels.

SPECIMEN COLLECTION AND HANDLING
Only serum specimens should be used in this procedure. Grossly hemolyzed, lipemic or microbially contaminated specimens may interfere with the performance of the test and should not be used. Store specimens at 2°- 8°C for no longer than one week. For longer storage, serum specimens should be frozen. Avoid repeated freezing and thawing of samples.
PROCEDURE
Procedural Notes
• Carefully read the product insert before starting the assay.
• All dilutions of the patient samples should be prepared prior to starting with the assay.
• Let patient specimens and test reagents equilibrate to room temperature before starting with the test procedure. It is suggested that reagents be left on the bench out of the box for 30 minutes prior to use. Return all unused specimens and reagents to refrigerator immediately after use.
• Remove required microwell strips from the pouch and carefully reseal the pouch to prevent condensation in the unused wells. Return pouch immediately to refrigerator.
• Good washing technique is critical. If washing is performed manually, adequate washing is accomplished by directing a forceful stream of wash buffer with a wide tip wash bottle across the entire microplate. An automated microplate washer is recommended.
• Use a multichannel pipette capable of delivering 8 or 12 wells simultaneously. This speeds the process and provides more uniform incubation times.
• For all steps, careful control of timing is important. The start of all incubation periods begins with the completion of reagent addition.
• Addition of all samples and reagents should be performed at the same rate and in the same sequence.
Test Method
Step 1 Let all reagents and specimens equilibrate to room temperature.
Step 2 Label protocol sheet to indicate sample placement in the wells. It is good laboratory practice to run samples in duplicate.
Step 3 For a qualitative determination use Calibrator D (yellow cap) only.

Step 4 Prepare a 1:101 dilution of the patient samples by mixing 5 µl of the patient sera with 500ul of Serum Diluent.
Step 5 Remove the required microwells from pouch and return unused strips in the sealed pouch to refrigerator. Securely place the microwells into the extra provided holder.
Step 6 Pipette 100 µl of Ready to use Calibrators, Positive and Negative controls and diluted patient samples (1:101) to the appropriate microwells as per protocol sheet.
Note: Include one well which contains 100 µl of the Serum Diluent as a reagent blank. Zero the ELISA reader against the reagent blank.
Step 7 Incubate 30 minutes (± 5 min) at room temperature.
Step 8 Wash 4x with wash buffer. For manual washing, fill each microwell with reconstituted wash buffer. Discard the fluid by inverting and tapping out the contents of each well or by aspirating the liquid from each well. To blot at the end of the last wash, invert strips and tap the wells vigorously on absorbent paper towels. For automatic washers, program the washer as per manufacturer’s instructions.
Step 9 Pipette 100 µl of Conjugate into microwells.
Step 10 Incubate 30 minutes (± 5 min) at room temperature.
Step 11 Wash all microwells as in Step 8.
Step 12 Pipette 100 µl of Enzyme Substrate into each microwell in the same order and timing as for the Conjugate.
Step 13 Incubate 30 minutes (± 5 min) at room temperature in the dark.
Step 14 Pipette 100 µl of Stop Solution into each microwell using the same order and timing as for the addition of the Enzyme Substrate. Read absorbance values within 1 hour of adding Stop Solution.
Step 15 Read absorbance of each microwell at 450 nm using a single or at 450/630nm using a dual wavelength microplate reader against the reagent blank set at zero absorbance.
Quality Control
Calibrators, Positive and Negative Controls and a reagent blank must be run with each assay to verify the integrity and accuracy of the test. The absorbance reading of the reagent blank should be

RESUMEN Y EXPLICACIÓN
Los anticuerpos contra la anti-LDL oxidada están relacionados con determinadas condiciones clínicas como la aterosclerosis, la estenosis carotídea y alteraciones en el funcionamiento de la arteria coronaria. Los anticuerpos contra la anti-LDL oxidada también pueden constituir un elemento de consideración en enfermedades como la diabetes y la endometriosis.1-5
La hipercolesterolemia está estrechamente asociada al riego de aterosclerosis.4,6 El colesterol acumulado en la placa aterosclérotica es inicialmente consecuencia de la lipoproteína de baja densidad (LDL). La oxidación de la LDL se considera como evento crítico en el proceso aterogénico.7 Los radicales libres como el superóxido y el óxido nítrico (-O2, NO) generados en las reacciones biológicas del cuerpo contribuyen a la oxidación de la LDL.8,9 Se ha demostrado que el óxido nítrico radical oxida la apolipoproteína B, un componente de la LDL. De igual modo, las lipo-oxigenasas y los oxidantes como el peroxinitrito pueden oxidar las mitades lípidas en la LDL. Se ha demostrado que la LDL oxidada y la LDL no nativa son asimiladas por receptores recaptadores en monocitos, células de musculatura lisa y macrófagos en los vasos sanguíneos. La oxidación de LDL aumenta la afinidad de la LDL oxidada con los receptores acetilos de macrófagos.10 Puesto que esta vía de absorción de LDL oxidada no está regulada, se forman macrófagos cargados de lípidos (células espumosas). Se ha demostrado la presencia de anticuerpos contra la LDL oxidada y sus complejos en el lupus eritematoso sistémico, la diabetes, las trombosis arteriales, la hipertensión, etc.
PRINCIPIOS DE PROCEDIMIENTOS
La prueba se lleva a cabo como un inmunoensayo de base sólida. Los micropocillos se revisten de antígeno de LDL oxidada y posteriormente se realiza una fase de bloqueo para reducir la fijación no específica de proteínas durante la realización del ensayo. En los pocillos recubiertos de antígeno se encuban controles, calibradores y el suero del paciente con el objetivo de permitir que anticuerpos específicos en las muestras se fijen al antígeno. Los anticuerpos libres y otras proteínas de suero se eliminan mediante el lavado de los micropocillos. Los anticuerpos fijados se detectan añadiendo a los micropocillos un conjugado anti-IgG humano con enzima. El conjugado no fijado se elimina mediante un lavado. Luego se añade un sustrato específico de enzima (TMB) a los pocillos y se detecta la presencia de anticuerpos mediante un cambio de coloración producido por la conversión de la TMB en un producto de reacción coloreado. La reacción se detiene y la intensidad del cambio de color, la cual es proporcional a la concentración de anticuerpos, es leída por un espectrómetro a 450 nm. Los resultados se expresan en unidades ELISA por milímetro (EU/ml) y se consideran como positivos o negativos.
REACTIVOS
Almacenamiento y preparación
Almacenar todos los reactivos a una temperatura de 2 a8°C. No congelar.
No utilizar si el reactivo no está transparente o si existe precipitado. Todos los reactivos deben llevarse a temperatura ambiente (20-25°C) antes de su uso.
Reconstituya el tampón de lavado a 1 litro con agua destilada o desionizada. Cuando se almacene a una temperatura de 2 a 8 °C, el tapón de lavado reconstituido se mantiene estable hasta la fecha de vencimiento del kit. Las tiras de micropocillos revestidos se utilizan una sola vez.
Precauciones
Todos los derivados de humanos, se han sometido a pruebas exigidas por la FDA para la detección de HBsAg, HCV, HIV-1 y 2 y HTLV-I. Todas ellas han dado negativas. RECOLECCIÓN DE MUESTRAS Y MANIPULACIÓN
En este procedimiento sólo deben emplearse muestras de suero. Las muestras hemolizadas o con contaminación microbiana o lipémica no deben utilizarse ya que pueden afectar el rendimiento de la prueba. Almacene las muestras a una temperatura de 2°- 8°C por un período que no debe exceder una semana. Para almacenamientos de más tiempo, deben congelarse las muestras de suero. Evita la congelación y descongelación repetida de las muestras.
PROCEDIMIENTO
Notas de procedimiento
• Lea el prospecto atentamente antes de comenzar el ensayo.
• Antes de comenzar el ensayo, deben prepararse todas las diluciones de las muestras de los pacientes.
• Deje que las muestras y los reactivos se equilibren a temperatura ambiente antes de comenzar con la prueba. Se recomienda que los reactivos se dejen en el banco, fuera de la caja durante 30 minutos antes de utilizarse. Vuelva a colocar todas las muestras y los reactivos no utilizados en la nevera inmediatamente después de su empleo.
• Retire las tirillas necesarias de la bolsa y vuelva a sellarla con cuidado para evitar la condensación de los micropocillos no utilizados. Coloque la bolsa inmediatamente en la nevera.
• Una buena técnica de lavado es esencial. El lavado manual óptimo se logra vertiendo un torrente fuerte de tampón de lavado por toda la microplaca, utilizando un frasco de boca ancha. Se recomienda una microplaca automatizada.
• Utilice una pipeta multicanal capaz de servir a 8 o 12 pocillos a la vez. Esto acelera el proceso y ofrece tiempos de incubación más uniformes.
• En todas las etapas es importante un control cuidadoso de los tiempos. El inicio de todos los períodos de incubación comienza al terminar de añadir el reactivo.
• La incorporación de las muestras y los reactivos debe realizarse con el mismo ritmo y en la misma secuencia.
Método de prueba
Paso 1 Dejar que todos los reactivos y muestras se equilibren a temperatura ambiente.
Paso 2 Marque las fichas de protocolo para indicar la ubicación de la muestra en los pocillos. Procesar las muestras por duplicado constituye una buena práctica de laboratorio.

Paso 4 Prepare una disolución 1:101 de la muestra del paciente mezclando 5 µl del suero del paciente con 500ul de diluente de suero.
Paso 5 Retire los pocillos necesarios de la bolsa y devuelva a la nevera las tirillas no utilizadas dentro de la bolsa sellada. Coloque bien los micropocillos en el soporte adicional que se suministra.
Paso 6 Siguiendo las fichas de protocolo, vierta con la pipeta en los correspondientes micropocillos 100 µl de calibradores listos para el uso, controles positivos y negativos y las muestras diluidas del paciente.
Nota: Incluya un pocillo que contenga 100 µl del diluente de suero como blanco de reactivo. Lleve a cero el lector ELISA en relación con el blanco de reactivo.
Paso 7 Incubar por 30 minutes (± 5 min) a temperatura ambiente.
Paso 8 Lave 4x con tampón de lavado. Para el lavado manual, llene cada micropocillo con tampón de lavado reconstituido. Deseche el fluido invirtiendo los micropocillos y dándole ligeros golpes o aspirando el líquido de cada pocillo. Para secar después del lavado, invierta las tirillas y seque bien los pocillos con toallas de papel absorbente. Para el lavado automático, programe la lavadora según las instrucciones del fabricante.
Paso 9 Vierta con la pipeta 100 µl de conjugado en los micropocillos.
Paso 10 Incubar por 30 minutes (± 5 min) a temperatura ambiente.
Paso 11 Lave todos los micropocillos como en el paso 8.
Paso 12 Vierta con la pipeta 100 µl de enzima sustrato en cada micropocillo en el mismo orden y con el mismo ritmo con que trabajó el conjugado.
Paso 13 Incubar por 30 minutes (± 5 min) a temperatura ambiente en la obscuridad.
Paso 14 Vierta con la pipeta 100 µl de solución quelante en cada micropocillo en el mismo orden y con el mismo ritmo con que agregó la enzima-sustrato. Lea los valores de absorción después de 1 hora de haber añadido la solución quelante.
Paso 15 Lea la absorción de cada micropocillo a 450 nm con un lector de microplaca simple, o a 450/630nm con un lector de longitud de onda dual, en relación con el blanco de reactivo establecido a una absorción cero.
Control de calidad
