The underlying functional abnormality in type 2 diabetes is impaired insulin secretion. This is responsible for many of the associated metabolic abnormalities.22 In healthy individuals, the increasing blood glucose that occurs while eating stimulates pancreatic β-cells to secrete insulin. This occurs in two phases – a rapid early phase and a prolonged late phase. The early phase of insulin secretion begins as soon as blood sugar levels begin to rise, and generally lasts for around 10 minutes. The second phase of insulin secretion then occurs and continues for as long as blood glucose levels remain elevated.23 Early-phase insulin secretion is essential for ensuring that the rapid increase in blood glucose levels that occurs during a meal is maintained within normal limits. It is important to realise that in type 2 diabetics it is this early phase of insulin release that is impaired (Figure 2).24 Because of this, type 2 diabetics are unable to control their mealtime blood glucose levels, resulting in the occurrence of glucose spikes.25

Legends abound about the origins of acupuncture. One story is that acupuncture was discovered observing that soldiers wounded by arrows and spears recovered from ailments on other parts of their bodies. Trial and error over many centuries evolved into a refined and detailed clinical methodology based on this system of correspondences. Another tale holds that semidivines sages mapped the pathways of Qi and chronicled the effects of ingesting herbs. What was invisible to the ordinary eye was visible to their enlightened perception.
By eliminating congestion and activating circulation of Qi, acupuncture interrupts and disorganizes patterns of illness. For example, a throbbing, hot full feeling in the head accompanied by coldness in the hands and feet suggest an excess of Qi in the head and a deficiency in the extremities. By stimulating points on the hands and feet, congested Qi and Blood are dislodged from the head and distributed throughout the body. The headache retreats, and a comfortable sense of warmth and connectedness pervades the body.
Acupuncture is most effective when functional disturbances have not yet developed into organic or structural impairments. Migraine headaches are more responsive to treatment that pain due to a brain tumor. However, in degenerative disorders like the joint deformities of arthritis or the chronic inflammation of nerves and muscles, frequent long-term treatment can bring about relief of pain and sometimes impressive recovery of function.

In myths from around the world, men and women have searched for an elixir that will bring protection from suffering. Buddhism’s answer is mindfulness. How does mindfulness work?
If you’ve ever seen film Gorillas in the Mist, you know about of Dian Fossey, the courageous field biologist who befriended a tribe of gorillas. Fossey had gone to Africa to continue the work of her mentor George Shaller, a renowned primatologist who had collected more intimate and compelling information about gorilla life than any scientist before. When his colleagues asked how he was able to learn so much about these shy and elusive creatures, he attributed it to one simple thing: he didn’t carry a gun.
Previous generations of biologists had entered the territory of these large animals with the assumption that they were dangerous. So the scientists came with an aggressive spirit, large rifles in hand. The gorillas could sense the danger around these rifle bearing men and kept a far distance. By contrast, Shaller—and later Fossey—entered their territory without weapons. They had to move slowly, gently, and, above all, respectfully toward these creatures. In time, sensing the benevolence of these humans, the gorillas allowed them to come right among them and learn their ways. Sitting still, hour after hour, with careful, patient attention, Fossey finally understood what she saw. As the African-American sage George Washington Carver explained, “Anything will give up its secrets if you love it enough.”
