Izvorni tekst - engleski This deposition shall be taken upon oral examination before an official court reporter or other officer authorized by law to take depositions and is being taken for the purposes of discovery, for use at trial, or for any other purpose allowed under the Florida Rules of Civil Procedure, the Uniform Rules of Procedure, and the Rules of the Florida Public Service Commission. The deponent is requested to bring and make available for inspection those documents identified in Exhibit A attached hereto.
EXHIBIT A
1. All documents in your possession, custody or control upon which you have relied on in any manner in forming your opinions in this matter, including, but not limited to , medical records, depositions, pleadings and other similar documents.
2. A copy of any notes written by you which refer to relate in any fashion to the child John Doe
3. All materials provided to you by the Dade County Department of Social Services which you have reviewed and upon which you have relied in any manner in forming your opinions in this matter, including, but not limited to, any chronologies or summaries.
4. All reports, whether in formal or draft form, made by you referring or relating in any way to the issues in this case.
5. All research completed or done by you or at your direction, related in any way to the issues involved in this case.
6. Produce a complete list of all cases in which you have given a deposition and/or trial testimony, including the full case caption, court file number, county and state in which the case was pending, a summary of the allegations in the case, the opinions you rendered, whether you testified and on whose behalf.
Prijevod - srpski Ova pismena izjava mora da se uzme nakon usmenog ispitivanja pred sudskim zapisničarem ili drugim službenim licem koje je zakonom ovlašćeno da uzima pismene izjave, a uzima se u svrhe istrage, upotrebe tokom suđenja ili bilo koju drugu svrhu koja je dozvoljena po Pravilima parničnog postupka Floride, Jedinstvenim pravilima postupka i Pravilima Floridske komisije za javnu službu. 4. Svi izveštaji, bez obzira da li su u obliku zvaničnih izveštaja ili nacrta, koje ste sastavili, a koji se odnose ili su na bilo koji način u vezi sa predmetom ovog slučaja.
